  1. 1. Please visit the USDA Pet Travel website to get the most up to date requirements for pet travel to designated country. https://www.aphis.usda.gov/aphis/pet-travel

  2. 2. Review and prepare your documents/requirements before your appointment (vaccines, testing, microchip).

  3. 3. Rabies Vaccination: If the vaccine was not performed at our clinic, please make sure you have a rabies certificate that includes name of vaccine, manufacture, lot/serial number and date given.

  4. 4. IF the country requires USDA State Veterinarians original signature please provide a NEXT DAY AIR pre-paid label from Fedex addressed back to yourself.

  5. 5. Make sure to check with your airlines for any requirements they have (veterinary health form, travel deceleration for weather or cargo pets).

  6. BEFORE your pets appointment provide records. Please email or fax us with destination address and local phone number and your USA address if different from our file at least 24 hours before your appointment. Also tell us if you are traveling on the same plane as the pet, pet is flying by itself or if a designated person (not owner) is flying with the pet.
